I would be interested in your indicators/tracking tool also.  We do not have a database to track where our qi cases go (ie peer review, trauma systems) but need one badly. We use a combination of the ACS indicators and some custom qi indicators.  One that we track regularly is neurosurgical management issues like no ICP monitor in a pt with TBI and GCS<8, differences in management recommendation between the neurosurgical residents and the attending and lack of neurosurgical attending seeing the pt.  At our institution, the neuro residents come from another residency.  They practice VERY independently and work for all the attendings...This can get rather challenging at times.
