I will scan our social work notes when I get home this evening and then post them on the list serve tomorrow.
I think they are pretty good to summarize the discharge plan. Our social workers also document on the progress notes in the chart. They don't use SAOP or CHART - they just do a narrative.

Our social workers write a narrative note when they interact with patients and their families. We have nurse case managers who do the discharge planning who do computerized documentation and print their notes including an admission screening for independence & d/c needs.
