As a matter of fact I just created one as a "nursing" peer review. There are no names - of either the reviewer / or the reviewee. We are a union based facility and there were some concerns about this being a punitive thing.

So... I have promised this only goes in the QI folder in the trauma office.

The goal of this form - I will give each ED nurse a copy of the form and the chart to review. I will use the schedule book so that all nurses get the opportunity to review a trauma chart. I am hoping that by reviewing other's charts that

        a. they will see areas that they are weak
        b. that during designation review - that I am actively working as is the whole ED nurse            population on documentation.

        c. then I plan on doing a documentation competency in May -- from our nurse risk manager.
