My name is Birgitte Soehus and I am working as a Trauma Coordinator at
Rigshospitalet, Copenhagen.

I am presently working on an article covering the communication between
trauma personell and the patient when doing the initial assessment and also
covering the tools we use in this regard. All in that short period of time,
which can help make the patients experience as positive as possible.

And therefore I would like to have any input or comments you might have in

We also use a tri-fold form and ordered clipboards that are actually sized
just for forms such as this.  We have four major trauma resuscitation rooms
and we only keep these special clipboards in those rooms.  It seems to work
out great because we seldom lose any of the documentation.

As a matter of fact I just created one as a "nursing" peer review. There are
no names - of either the reviewer / or the reviewee. We are a union based
facility and there were some concerns about this being a punitive thing.
So... I have promised this only goes in the QI folder in the trauma office.

The goal of this form - I will give each ED nurse a copy of the form and the
chart to review. I will use the schedule book so that all nurses get the
opportunity to review a trauma chart. I am hoping that by reviewing other's
charts that
        a. they will see areas that they are weak
        b. that during designation review - that I am actively working as is
the whole ED nurse         population on documentation.
        c. then I plan on doing a documentation competency in May -- from
our nurse risk manager.

This is the current one I use-we change the content yearly. I review all
trauma alert documentation and provide direct feedback to the ED nurses.
(only about 270 trauma alerts/annually).A copy of the form is sent to the ED
Unit Director and she uses these for annual performance evaluations.  I use
the bolded items for trauma pi audit filters and report this to the ED and
Trauma Committee.

Does anyone have a Audit Tool for trauma flowsheet documentation.  I am
in the process of developing one and would like to see some samples.
Anything would help.

Jennifer, here is the tool that we utilize.  These audits are done
quarterly
by the ER staff themselves to make them accountable for their
documentation.

Ours is tri-fold also, we had a clipboard made out of plexiglass that
accomodates that size.

I am looking for advice from anyone who has had an ACS Verification
Review Visit.  I was wondering what you do, if anything, to prepare the
nursing staff for their visit.  Traditionally for JACHO visits, everyone
reviews policies, emergency plans etc.   I don't know if there are
standard type questions the reviewers like to ask the staff that I could
help prepare them.  Any advise would be greatly appreciated as this is
my first time going through this and we have a lot of new nurses in our
ED who have never gone through a visit before either.

Deb, I meet with the Nurse Directors and Nurse Managers for all areas
that see trauma patients:  ED, ICU, OR, Trauma Unit, PACU, etc.  I also
made sure they knew what date the reviewers where coming.  For my visit,
they were very interested in nursing credentials and what kind of
orientation the staff gets to care of the trauma patient.  They were
also interested in who follows patients in the ICUs, Attending presence
in the ICUs.  When I got the schedule for the day of what areas the
reviewers wanted to see I called the units to give them a heads up that
we were coming.  One last thing, the reviewers split up and I went to
some areas with one of them and my Medical Director went with the other
one.  Hope this helps.

There is a document titled "Resource Document for Nursing Care of the Trauma Patient" Copyright 1992, Trauma Nursing Coalition. It is the older version
of a current book, however, it is packed full of typical questions asked of
your ED, OR, ICU, etc. staff.  It was very helpful for me.  I've used it
twice already.  Kirstie
