In response to the clipboard for a tri-fold. I purchased a large board
from a office supply store and some large clips and our maintainance
dept build ours to fit our trauma record. Recently we changed to a 3
page form (duplicate copy) and use a normal size clipboard with dividers
between the pages which seems much easier.
