Admin made it happen. The ED pointed out the obvious potential lawsuits and they took action!

This is an interesting question. We have had more than one he said / she said and I am still having difficulty getting this in writing. This has been to the top of the hospital chain of command. How did you get them to make this happen?

We have our radiologists give a written initial read, they can fax it to the ED if need be but it must be written. We had a few he said/ she said situations that made this necessary. We can then compare that with the final read. Any discrepancies are brought to the attention of the chief radiologist immediately and also dealt with through M&M.

Does anyone have a good system for tracking misreads by Radiology?
