I believe the American Academy of Pediatrics has an algorithm for children
2-20years in Vol.104 No.6 December 1999 page 1413. For Children < 2 there
is an Algorithm on page 990 of Pediatrics Vol. 107 No.5 May 2001 (2
different magazines)
