According to the UB-92 Manual, the reference is to the level of your facility. 
See point number 6 on the attached page from the manual.

I am looking at the CPT information from The American Medical Association
with subcategory codes listed as mentioned below:
 
680 - not used
681  - Level I
682 - Level II
683 - Level III
684 - Level IV
689 - Other trauma response
 
Would you mind sharing how you are interpreting the "Level" in this code?  I
have heard it two ways - 

*       one is the "Level" is the Level of your Trauma Center
*       second is that "Level" refers to the level of care the patient
received depending upon severity
