Robynn,
We are very confused as well.  I believe we are using 681 for all of our
trauma patients as we do not have a tiered system.  All of our trauma
patients arrive as major trauma patients and therefore our finance/business
office is using 681.  I was under the impression that we should be using 682
as we are a Level II trauma center.  Very confusing.  I would appreciate
clarification too. Thanks for bringing up the question.

I am looking at the CPT information from The American Medical Association
with subcategory codes listed as mentioned below:
 
680 - not used
681  - Level I
682 - Level II
683 - Level III
684 - Level IV
689 - Other trauma response
 
Would you mind sharing how you are interpreting the "Level" in this code?  I
have heard it two ways - 

*       one is the "Level" is the Level of your Trauma Center
*       second is that "Level" refers to the level of care the patient
received depending upon severity
