We do not keep original certificates on file nor do we have a good system
for this. Some of the files are kept in the medical staff office, some are
kept by the individual's secretaries and some are kept by the individuals.
Our chief of Trauma does get CME info from ortho and neuro as well but
makes the Emergency Medicine Dept. responsible for gathering their info.
The secretary for the Chief helps with this process alot (gathering the
info, nagging the surgeons to comply) and my registrar/secretary helps
with the spreadsheet. I have attached a sample spreadsheet for you to see
what I use (it is very simple). I did one of these for each physician. We
used this for our state survey about 1 year ago and it satisfied them.
