We also have initiated a break from sedation each morning. We also try to
have it coordinated with then weaning attempt if they meet criteria each
day. It is to be restarted if the patient has clinical indications of
distress, this is usually 15-30 minutes on most patients. If the best
response does not need as much sedation to keep at a MAAS of 3 then a lesser
dose is to be used. April

We do not keep original certificates on file nor do we have a good
system
for this. Some of the files are kept in the medical staff office, some
are
kept by the individual's secretaries and some are kept by the
individuals.
Our chief of Trauma does get CME info from ortho and neuro as well but
makes the Emergency Medicine Dept. responsible for gathering their
info.
The secretary for the Chief helps with this process alot (gathering
the
info, nagging the surgeons to comply) and my registrar/secretary
helps
with the spreadsheet. I have attached a sample spreadsheet for you to
see
what I use (it is very simple). I did one of these for each physician.
We
used this for our state survey about 1 year ago and it satisfied them.
Hope this helps.

Hi Lynn,
Does this include orthopedic surgeons, emergency medicine and
neurosurgeons also? Do you get copies of the original certificates?
I'm told that a site visitor asked for copies during a visit. Do you
have secretarial support for this task? Thank you. Liz Turney

Our chief of trauma requires all trauma surgeons to report CME to him
on a
yearly basis. I then keep an excel spreadsheet with the info.

We are trying to impplement a sedation/analgesia protocol in our unit right
now. We would like all physicians to use the accompaning order set on all
patients in the ICU. The policy and order set suggest a daily holiday but
do not mandate it. If it is safe for the patient we would like the nurse to
hold sedation for one hour each am (day shift before rounds is the best
time), assess neurological status and then re-start the infusion at the
lowest tolerated rate and titrate up according to the Ramsey scale.
