The Hippa police have just written me a ticket!
When your registrar finds something that is in the chart but not coded by
'hospital coder' (for example - an Xray report shows the presence of a
pelvic fracture - but it wasn't mentioned in progress note or dictation -
and so doesn't meet criteria for inclusion by hospital coders) does your
registrar include it in your trauma database?
