At our facility, if a trauma patient arrives and any uncrossmatched red cell
units are transfused (O neg or O pos), a retrospective crossmatch sample
must be done. According to the standards of the American Association of
Blood Banks (AABB), a crossmatch must be performed on every unit of red
cells transfused to a patient, even if it is performed after the
transfusion.  A common scenario for when a retrospective crossmatch is
required is with a trauma patient receiving uncrossmatched red cells because
of the urgency of the situation.  The retrospective crossmatch sample must
be drawn as early as possible in the transfusion sequence, ideally prior to
the first transfusion. Another patient sample for crossmatch should be sent
by the patient care area as soon after the transfusion as possible.  This
might be the rationale behind this requirement at your facility.

At our facility, if a patient comes in as a "DOE" we will get a type and
cross using the "DOE" name.  Once the patient is identified, we get another
T&C sample using the new name (the hospital number doesn't change) to
confirm the patient's type and update the new information with our blood
center.
