Short Survey;
Does your facility  use three categories for assigning
preventability of
deaths:
        Nonpreventable / potentially preventable / preventable??

or four categories:
        Nonpreventable / possible preventable / probably
preventable /
preventable??

We draw one specimen - either drawn by the ED staff or the
lab person that
arrives  - The person that draws it is expected to label it
and assure it is
delivered to the lab person = Why do they have this policy
at your
facility.??-
