We also just completed a Level I site visit this week and the reviewers wanted
to see the Level (for tiered response) of trauma, the level of physician
responding as well as the time of activation/response and names/signatures on
the flow sheet. Hope that helps.

We just completed a site visit and the ACS visitors looked at the time the
trauma teams were activated and the response times of the members of the team
on each chart they reviewed. We have this info on the trauma flow sheet.

We have it on ours. The ACS looks at that during inspection.

I am in the process of revising our Trauma Flowsheet. It would be helpful to
know how many Level II ACS trauma centers have Surgeon call/response times
and activation team response times on the trauma flowsheet itself. Has
anyone received feedback from ACS regarding this issue in the past? Do you
know what is preferred? Thank you in advance for responding.
